What is Chronic Disease Management?
Chronic disease management is exactly what it sounds like – it’s a method of controlling chronic diseases or long-term illnesses to enhance a patient’s health and overall quality of life. Chronic disease management looks different for everyone depending on their condition and their individual needs. Our providers are happy to work with you on an individual basis to ensure all of your specific medical needs are met.
Chronic Conditions Treated
While treatment and management plans may vary, common conditions for which we offer chronic disease management services include the following:
High blood pressure occurs when the force of the blood pushing against the walls of the arteries is consistently too high. High blood pressure can occur due to genetic factors but is also often brought on as a result of an unhealthy diet and lifestyle.
Our chronic disease case managers can work with you to get a better handle on your high blood pressure with nutrition education, regular blood pressure screenings, blood pressure medication management, and more.
Cholesterol is a natural and healthy part of our body’s system. However, when cholesterol levels get too high in the blood, they can leave fatty deposits within the blood vessels. This can lead to a host of problems, including increased risk of heart attack and stroke.
High cholesterol can be caused by genetic factors but is most commonly associated with a high-fat diet. Our chronic disease case managers can provide you with nutrition education, regular cholesterol level tests, cholesterol medication management, and more.
Diabetes is a condition that affects the body’s ability to process and store sugars. There are two types of diabetes – diabetes type I and diabetes type II.
To help assist you in managing your diabetes, our case managers may offer nutrition education, assistance in using an insulin pump or continuous glucose monitor, aid in medication management, and more.
Other chronic diseases we assist with include:
- Arthritis
- Asthma
- Chron’s disease
- Hyperthyroidism
- Hypothyroidism
- Osteoporosis
- More
It’s important to keep in mind that the lists above are by no means comprehensive. We are proud to offer our individualized treatment and chronic disease management plans to a wide range of men, women, and children.
