As a recognized Patient Center Medical Home, G.A. Carmichael Family Health Center provides comprehensive primary care and preventive medicine for all ages – from infants to seniors. Chronic Disease Management
Chronic diseases are conditions that affect patients in the long-term and require ongoing care and attention. At G.A. Carmichael Family Health Center, quality care is our primary focus. This includes the management of common chronic diseases, such as:

Primary & Preventive Care
Primary care means more than having a physician you can go to when you’re sick. It means having a provider you can rely on for routine visits to prevent illness before it develops. G.A. Carmichael is here for you whether you’re in need of a physical for work or need your child to get their immunizations before school.
Physicals are general check-up examinations that allow your provider to get a full view of your overall health. Your physical will likely be comprised of two parts: a medical history review and a physical examination. We provide physicals for all occasions, including school, marriage, admissions to college, and more.
Oftentimes, physicals are required prior to a child beginning a sport or an adult entering a new job that has a focus on manual labor. These physicals differ from a traditional physical, as your ability to carry out certain physical activities will be assessed during your exam.
Immunizations are often required periodically for children, whether it’s for admission into school or as a general precaution. We are happy to offer all common immunizations to children and adults alike – including annual flu shots and COVID-19 vaccinations.
Cancer screenings are a common preventive care measure that we recommend to adults over the age of 30. These screenings include screenings for breast and cervical cancer, as well as prostate cancer. Ask your provider if you are in need of a routine cancer screening.
